DR. MOHAN CHOUDHARY MEMORIAL FOUNDATION a non-government organization is located in Darbhanga, Bihar. The foundation is registered as a Public Charitable Trust under Indian Trust Act 1882 with registration number 67/4. The registration was done on July 22, 2015.

LATE DR. MOHAN CHOUDHARY MEMORIAL FOUNDATION (1960-2015) was a great social worker who devoted his life for the cause of physically challenged persons. He himself was physically challenged. He chosen to lead life like a yogi, remained unmarried. He had love and affection for all sectors of the society. He was a learned personality in the field of mathematics and completed his doctorate degree. He was a good teacher of mathematics. He used to teach Math to the students of high schools and colleges. He was employed as a teacher in state government for a short period of time. He took his last breath on 16th of April, 2015 due to chronic muscular dystrophy followed by sudden cardiac arrest. During his lifetime, Dr. Choudhary has numerous achievements to his credit. The physically challenged people of North Bihar still remember the services of The Lifeline Express parked at Muzaffarpur which carried out more than 350 intricate surgical operations free of cost & help bringing the beneficiaries in leading nearly normal life. His leadership and credibility made him successful in mobilizing support of local and expert surgency from different parts of the country to render their services in the Lifeline Camp. The local surgency benefitted by assisting the Expert surgeons who started replicating the techniques locally. He had much concern for education & empowerment of the physically challenged persons. He started aspecial school for the physically challenged children. Recognizing its achievement & credibility the Ministry of social justice, Govt. of India is extending financial support.
The school is located in the Muzaffarpur Industrial Estate, Bela. He started empowering the people by providing skill training in Computer Application, Screen Printing, Lac Bangla Making, Food Preservation etc. The Programme was financially supported by a Canadian NGO namely NDRF. A Brishsh NGO- ABLIS helped him in sesitising the PH people &the Govt. machinery to facilitate them in getting their due rights. Before his sudden demise he was able in carrying out survey, forming groups & developing local leadership & sensitizing the PH in 4 blocks of Muzaffarpur Dist. Under PACS Project successfully. His fighting spirit & concern for cause of the PHs did not permit him to think for him self & behind him & his efforts at each & every step. The family, even after his demise continues helping for his dreams come true.

✓ To run, own, manage, administer Nursing Home, Diagnostic Centres, Scan Centers, Hospitals, clinics, Dispensaries, Maternity Homes, Child Welfare and Family Planning Centers, Clinical, Pathological Testing laboratories, X-Ray and ECG Clinics, Eye-infirmaries.
✓ To maintain, Manage and supervise Hospitals, Nursing Homes, Relief and rehabilitation Centers and any other ancillaries services.
✓ To provide, encourage, intimate or promote facilities for the discovery, improvement or development of new methods of diagnosis, understanding and prevention and treatment of diseases.
✓ To establish, run, support, develop and maintain hospitals, Medical and Para Medical institutions, Medical Colleges, libraries, reading room, universities, laboratories, research other institution of the link nature in India, for use of the students and the staff and also for the development and advancement of Health education and diffusion of knowledge amongst the public in general without any distinction as to caste, colour, race, creed or sex.
